Bij ahti kijken we met interesse naar verschillende nieuwsitems op het gebied van gezondheid, data en innovaties die van invloed zijn op zorg. Zo doen we nieuwe inzichten op en discussiëren we over mogelijke verbeteringen in de zorg. Afgelopen weken zagen we deze items in het nieuws langskomen en dit delen we graag met jullie.

  1. Zorg voor honderdduizenden patiënten moest wijken voor coronavirus – Volkskrant, 20-04-2020
    Honderdduizenden patiënten hebben de afgelopen maanden niet de zorg kunnen krijgen waar zij recht op hadden, met mogelijke gezondheidsschade tot gevolg. Dat blijkt uit een rapport van de Nederlandse Zorgautoriteit (NZa).

2. Ambtelijk advies: huidig zorgstelsel is onhoudbaar, politiek moet rigoureus snijden in zorgkosten – Volkskrant, 22-04-2020

De politiek moet rigoureus ingrijpen om te voorkomen dat de kosten van de gezondheidszorg in de komende twintig jaar verdubbelen. Die dringende oproep staat in een ambtelijke advies aan het kabinet, opgesteld vóór het uitbreken van de coronacrisis.

3. Gaat gereguleerde marktwerking tot 2022 de ijskast in? – Zorgvisie, 15-04-2020
De hiërarchische aansturing van de zorg die bij deze crisis hoort, zal pas verdwijnen als er een vaccin tegen het coronavirus is. ‘Als ziekenhuizen en MSB’s in de tussentijd heel constructief hun eigenbelang laten wijken voor het landsbelang, dan zal er daarna weinig steun zijn om de marktwerking weer terug in te voeren’, zegt gezondheidseconoom Xander Koolman. ‘Maar dat verwacht ik niet.’

4. RIVM: stijging zorgkosten kan leiden tot afname solidariteit – Zorgvisie, 20-04-2020
Naar verwachting blijven de zorguitgaven de komende decennia stijgen, ook na de piek van de vergrijzing rond 2040. Dat meldt het RIVM in een vandaag verschenen rapport. Deze stijging kan leiden tot een afname van de solidariteit in het zorgstelsel.

5. Zorgverzekeraars helpen bij versnelling digitale transformatie zorg – Zorgverzekeraars Nederland, 20-04-2020
De coronacrisis versnelt de digitale transformatie van de zorg. Er is veel bereidheid om sa¬men te komen tot doeltreffende oplos¬singen voor nu, die ook levensvatbaar zijn in de toekomst. Zorgverzekeraars spelen daarbij een belangrijke rol. “Maak als zorgaanbieder gebruik van de kennis en ervaring van zorgverzekeraars. Zorgprofes¬sionals willen zich, logischerwijs, bezighou-den met het verlenen van zorg. Het versneld oppakken van innovaties kan daarbij helpen. Zorgverzekeraars hebben mensen om daadwerkelijk de zorg te helpen; zij staan klaar om te helpen bij bijvoorbeeld implementatie- of financieringsvraagstukken”, zegt Menno Jansen, innovatiemanager bij CZ. Hij doet de uitspraak in een artikel dat maandag is verschenen in het tijdschrift ICT&Health.

7. Quantifying SARS-CoV-2 transmission suggests epidemic control with digital contact tracing – Science, 30-03-2020
The newly emergent human virus SARS-CoV-2 is resulting in high fatality rates and incapacitated health systems. Preventing further transmission is a priority. We analyzed key parameters of epidemic spread to estimate the contribution of different transmission routes and determine requirements for case isolation and contact-tracing needed to stop the epidemic. We conclude that viral spread is too fast to be contained by manual contact tracing, but could be controlled if this process was faster, more efficient and happened at scale. A contact-tracing App which builds a memory of proximity contacts and immediately notifies contacts of positive cases can achieve epidemic control if used by enough people. By targeting recommendations to only those at risk, epidemics could be contained without need for mass quarantines (‘lock-downs’) that are harmful to society. We discuss the ethical requirements for an intervention of this kind.

9. Projecting the transmission dynamics of SARS-CoV-2 through the postpandemic period – Science Magazine, 14-04-2020

It is urgent to understand the future of severe acute respiratory syndrome–coronavirus 2 (SARS-CoV-2) transmission. We used estimates of seasonality, immunity, and cross-immunity for betacoronaviruses OC43 and HKU1 from time series data from the USA to inform a model of SARS-CoV-2 transmission. We projected that recurrent wintertime outbreaks of SARS-CoV-2 will probably occur after the initial, most severe pandemic wave. Absent other interventions, a key metric for the success of social distancing is whether critical care capacities are exceeded. To avoid this, prolonged or intermittent social distancing may be necessary into 2022. Additional interventions, including expanded critical care capacity and an effective therapeutic, would improve the success of intermittent distancing and hasten the acquisition of herd immunity. Longitudinal serological studies are urgently needed to determine the extent and duration of immunity to SARS-CoV-2. Even in the event of apparent elimination, SARS-CoV-2 surveillance should be maintained since a resurgence in contagion could be possible as late as 2024.

10. ‘Het riool geeft eerder een signaal dan dat coronapatiënten zich melden’ – Het Financieel Dagblad, 15-04-2020
Het riool wordt gebruikt als een monitoringssysteem voor de verspreiding van het coronavirus. ‘Één zuiveringsinstallatie kan afvalwater opvangen van meer dan een miljoen mensen.’

11. Digitaal minister Taiwan over corona-apps: ‘Erken dat je de privacy een beetje moet schenden’ – Trouw, 17-04-2020
Taiwan is zeer succesvol in de aanpak van de coronacrisis. Naast hygiëne en afstand houden, helpt moderne technologie daarbij. Digitaal minister Audrey Tang vertelt hoe haar land dat doet.

12. Corona check-app van OLVG nu door iedereen te gebruiken – NOS, 21-04-2020
De Corona Check-app van het OLVG-ziekenhuis in Amsterdam is nu voor heel Nederland beschikbaar. In de app kunnen mensen medische gegevens invoeren en melden hoe ze zich voelen. Als daar een verdacht beeld uit ontstaat en wordt gedacht dat iemand besmet zou kunnen zijn met het coronavirus, dan neemt een arts contact op met de gebruiker van de app.

13. Steven van de Vijver en Kim van Wijck schreven een artikel in de Huisarts en Wetenschap over het DHoTS project waar ahti in 2019 mee is gestart samen met partners ROHA, Heart for Health ICT en Amsterdam UMC, locatie AMC – Academic Medical Center.

Zelfmonitoring van de bloeddruk geeft een betere risicoschatting van harten vaatziekten (HVZ) en draagt bij aan bloeddrukverlaging. Hoe kan de huisarts deze zelfmonitoring sturen en begeleiden? Een nieuwe zelflerende app moet dit vergemakkelijken.

We are a group of scientists based at the Big Data Institute at Oxford University with expertise in infectious disease epidemiology, medicine, virology, immunology, mathematical modelling, phylogenetics, behavioural economics, and ethics. We have redirected our usual research on stopping virus spread to address the new pathogen SARS-CoV-2 and contribute to stopping this deadly pandemic. We ask whether, from a mathematical point of view, it is possible to stop the epidemic. We come to the conclusion that the epidemic can be stopped if contact tracing is sufficiently fast, sufficiently efficient and happens at scale. We suggest that the best way to achieve this is by using a mobile app. Read our paper published in Science or jump straight into exploring the results on our interactive dashboard below. We have now developed an individual-based epidemic simulation to enable epidemiologists, app designers and policy makers to compare a variety of algorithms for digital contact tracing, with different assumptions about the epidemic, technology, demographics, and user engagement. Our model is open source and accessible here. We identified a safe and effective starting configuration for the UK that can be optimised to prevent transmission while minimising isolation. 2. The COVID-19 Health Systems Response Monitor (HSRM)
Potentially interesting, but still a few blanks in data.
The COVID-19 Health Systems Response Monitor (HSRM) collects and organizes information on how countries’ health systems are responding to the crisis. It currently includes information for most of the countries in the WHO European Region, including all EU Member States, and is updated regularly.
